For some plans, we are out-of-network, though we have applied to numerous private insurance plans including Medicare and Medicaid.  Due to our out-of-network status, we must ask for direct payment for services rendered.  Patients are welcome to apply for reimbursement from their insurance carrier and we can help with the paper work to facilitate this.  We urge our patients to check with their insurance plans, as some do not cover out-of-network treatment.

Ketamine and other acute pain infusions may not be covered by health insurance including Medicare and Medicaid.  Most other services, such as joint injections, nerve blocks, trigger point injections, the office history and physical exam may be covered although prior authorization is sometimes required prior to some interventions.  Please contact our office for more information on our financial policies.
